All forms must be completed before your child enters our care. All phone numbers and addresses must be kept up to date, so please notify us if there are any changes. 

All children enter on a two week trial basis to insure proper adjustment. During this trial period, either party may cancel without notice. After that a minimum of two weeks notice, or two weeks payment is required to withdraw your child from the program.

 
All children must be kept up to date with immunizations.

We are very concerned here at 4-KIDS Family Day Care about maintaining a stable and caring environment for your child and we need your cooperation to achieve this.

Part of this cooperation involves a financial commitment from you. Weekly payment is required year-round whether your child is in attendance or not. And there will be no credits for absences, illness, holidays, vacations, etc.

This payment policy will guarantee that your child's space is reserved for him/her when he/she is not present.

Parents are required to pay the weekly fee in advance This fee is due the morning of the first day of the week that your child attends daycare.
Weekly fees must be paid in advance when leaving on vacation. There will be a five dollar ($5.00)charge for each day that a payment is late per child. There will be a ten dollar ($10.00) charge for any returned checks. After three (3) returned checks all future payments must be made
in cash or by money order.
NO EXCEPTIONS!
